Voluntary Disclosure Program Application Form

Forms

Our Voluntary Disclosure Program (VDP) enables contractors to assess their own compliance and comply with the regulations without incurring penalties that would have resulted had they not voluntarily stepped forward.  As well, participation in the program keeps the contractors enforcement history off of the public registry, which helps preserve their reputation.

Find out more information about our Voluntary Disclosure Program.

Complete the Voluntary Disclosure Program application form below to be considered for entry into the program.
