Incident overview

A 2000kVA unit substation transformer was damaged beyond repair after a phase to ground fault in the secondary low voltage compartment. The outdoor substation supplies power to a community center and sustained the damage after a conductor wire was installed in a way that caused it to rub against a sharp surface inside the compartment. This friction wore down the wire's insulation, exposing the wire and leading to a short circuit.
