Purpose: This list is a supplement to Directive D-L4 110803 5 Rev.5 “Major and Minor Alterations.” It outlines situations where an alteration permit is required (“major” or “minor”) based on the scope of work, and indicates when an inspection is required (“x”). Shaded areas are not applicable and do not require an alteration permit or inspection. Note: For minor alterations where an inspection is indicated, the safety officer may use their discretion to perform a physical assessment (inspection) or accept a declaration without a physical assessment (inspection).
